Before fill-in the form please prepare the details of the staff and requirement as following:
1. Name, start date, and end date of the original contract.
2. End date of the extension period.
3. A regular extension means that the project end date is extended (changed to a later date) but no additional funds are added by the sponsor to cover the extension period.

4. An additional honorarium shall be a reasonable estimate of the anticipated expenses. Honorarium should be charged to charging code. If honorarium is taken from another source of code, it must be informed and provided.
5. An additional assignment to the contract including additional or changes of the terms of reference or scope of work from the original contract must be provided in the new terms of reference and should be agreed by both parties.
6. Changes to the contract modality including addition, removal, and/or reduction in the terms of reference from the original contract must be provided in new term of reference and should be agreed by both parties.
